Clinical management of patients' medications

Medication management is a strategy for engaging with patients and caregivers to create a complete and accurate medication list. A complete and accurate medication list is the foundation for addressing medication reconciliation and medication management issues.

Oversight of medication exchanges

There are strict regulatory guidelines regarding medication use so pharmacist consultation is critical. One of the most important services we provide is making sure nursing facilities are following established guidelines to avoid patient harm and financial penalties for the long-term care facility. Long-term care facilities are inspected by state agencies (ten specific departments may vary from state to state) on an annual basis or more often if a complaint or patient injury needs to be investigated. The department uses direction and follows rules for proper medication use in long-term care facilities. These are guidelines that we are very familiar with in order to monitor the facility properly.

Specialized medication packaging

United Specialty Pharmacy offers specialized medication packaging for their residents to:

  • Enhance the accuracy of medication administration and reduce errors
  • Enhance accountability of controlled substances to reduce drug diversion
  • Enhance the efficiency of medication management to save nursing time

This packaging also means there’s no need for multiple prescription bottles from various pharmacies. Single-dose blister cards give you a visual reference so you know exactly how many medications you have left. Each blister contains one medication. We can also provide multi-dose blister cards, organized by time-of-day and day-of-week. This gives you a visual indication of what to take and when. The easy-to-follow system helps you avoid missing or doubling up on doses.

Seasonal flu clinics

Studies have shown that pharmacists provide a very convenient and accessible option for seasonal flu vaccination, and that option is viewed favorably by patients. We can help you set up and administer the flu and other vaccines to improve public health by increasing vaccination rates, especially among high-risk patients, first-time or occasional vaccine recipients, and patients who may not otherwise have an opportunity to be vaccinated.
